La gestión clínica pretende restaurar los niveles de salud de los pacientes, en la medida de lo posible, mediante acciones que conciernen a pacientes individuales o grupos de pacientes, agrupados por una patología o procedimiento comunes. Se pretende favorecer la evolución de la práctica de la medicina, hacia una mayor orientación a las necesidades del usuario, el creciente fundamento de la evidencia y la interiorización del coste de oportunidad de las actuaciones.

Por ello, antes de pasar a desarrollar el proceso de descentralización de toma de decisiones, analizamos en profundidad dos aspectos que son muy relevantes en el proceso asistencial, como son la gestión del conocimiento y la variabilidad de la práctica clínica, puesto que sobre ellos va a recaer la posibilidad de avance y mejora de la eficiencia en la gestión del proceso asistencial.
