¿Debemos realizar un cribado sistemático de la preeclampsia (PE) en el primer trimestre?


La preeclampsia afecta aproximadamente al 2-5% de todos los embarazos, llegando hasta el 8% y es una causa muy importante de morbi-morbilidad materna y perinatal. En la última década, existe un gran interés en la detección temprana de la preeclampsia mediante el desarrollo de algoritmos para estimar el riesgo individualizado de las gestantes que pueden desarrollar PE. Así, se podría reducir la prevalencia de la enfermedad mediante la intervención farmacológica en el grupo de alto riesgo desde el primer trimestre del embarazo y, en caso de que se desarrollen complicaciones severas materno fetales por la aparición de la preeclampsia, minimizar los efectos (Henderson, Thompson et al. 2016).

Los diferentes cribados basados en características maternas (historia clínica, presión arterial, índice de pulsatilidad de las arterias uterinas y biomarcadores) propuestos, tiene una tasa de detección superior a la de la historia materna aislada, tanto para la PE precoz como la tardía (Akolekar, Syngelaki et al. 2011; Poon and Nicolaides 2014; O’Gorman, Wright et al. 2016)

La adición de diferentes parámetros biofísicos y bioquímicos a los factores maternos incrementan la tasa de detección de preeclampsia.
