Terapia de resincronización cardiaca. Fundamentos y selección de pacientes


La terapia de resincronización cardíaca (TRC) tiene como principal objetivo modificar la disincronía electromecánica que existe en pacientes con trastornos de la conducción. Se estima que el 20-30% de los pacientes con insuficiencia cardiaca (IC) en clase funcional III/IV tienen un trastorno de conducción intraventricular significativo (QRS 150 ms), lo que les hace candidatos al tratamiento con terapia de TRC. Optimizando la sincronización de la contracción ventricular conseguimos una mejora en la función ventricular, corrigiendo el proceso de remodelado ventricular en pacientes con IC.

La TRC es una alternativa terapéutica para pacientes con insuficiencia cardiaca que están recibiendo tratamiento médico optimizado y que no son candidatos al trasplante cardiaco de manera inmediata.

Revisaremos las bases en las que se fundamenta el beneficio de la TRC, los aspectos clave a considerar durante el implante, así como la selección de los candidatos.
